• A passport OR US government issued birth certificate with a raised seal and a photo ID.

• Non-US Citizens require a passport.

• You need to arrive in Miami, FL no later than 12:00 pm on 04/07/22 and leave no earlier than 12:00 noon 04/11/22. 
• Take a taxi, Uber, or Lyft to the Pier from the airport. 
• We highly encourage all travelers to purchase trip protection.

- A deposit of $200.00 per person is required at the time of booking. This deposit is nonrefundable. 
- The credit card provided on this form will be used for all future payments.
- Payments are processed directly through Xstream Travel.
